I tried to provide a variety of rewards that cater to different motivators. Some focus on giving students more of things they love (an item, activity, attention), and some take away less favorable things (homework, an activity or item, and for some, attention).

I recommend keeping a log (you can use what I provided at the end) to make sure each student gets at least one reward weekly. Some rewards benefit the whole class, but with choice or focus for the one rewarded, so hopefully it motivates the whole class, even when they don't all win something one day. Use discretion for things that give students access to items or activities they don't normally have access to, and choose which you use. Some students may hate reading, so don't give them the ones that involve sharing books aloud, as an example. You know your students.
