This resource was created in order to help Speech Language Pathologists target initial phonemes at the word level.

The h-insertion trick can be an extremely helpful strategy to use when children are stuck at the isolation level of a sound. By producing /h/ after the initial sound, the child continues their air flow, making it easier to produce the following vowel. This helps them to eliminate the prior habit of producing the incorrect sound.

Science of Reading Assessment Tool Tracking Sheet

I created a French SOR Assessment tool to cover all the sounds for students K to 3 in the French Immersion setting (Grade 4-6 Core French) for easy assessment and evaluation. Once I got started on my own testing, I realized I could use a quick look tracking sheet.

I learned how to make the boxes editable and hope it works out for you. (I did ask a few colleagues to try it out first). I am offering it FREE for now.

Use this sheet to assess your student counting to 50. Highlight missed/prompted numbers to see where mistakes are occurring.

This is a PDF Form with the Third Hundred Fry's Instant Words. As the student reads the words, the assessor touches the box next to the word if it is correct. The form automatically counts the number correct and displays it in the upper right corner. There is also a table to add date and number correct to see improvement over time.
