This contract includes: description of services, hours, contact information, attendance/no show policy, financial agreement, rates and packaged rates, confidentiality, professional records, code of conduct, and discharge/termination. This is a digital download for practitioners to edit, format, and utilize to begin their journey into private practice.

This resources is used to track supervision of Registered Behavior Technicians (RBT®) based on the requirements set forth by the Board of Applied Behavior Analysts®. The tracker should be used to ensure supervision meets the requirements set forth and includes a section for notes. Sample RBT Tracking is included.

Supervisor and RBT® should share responsibility in ensuring hours are accurately calculated. This tracker is automated in calculating percentage, but requires RBT® to determine the total amount of behavior analytic services they provided during the month. Hours logged in the excel sheets should only include hours in which supervision has been provided.
